One page checklist to be signed by each competing club prior to any match commencing. A requirement of League and its insurers. Form is to be returned to the League with the match day results envelope.
3. Umpire and Timekeeper Instructions
Printable pdf on for umpires and timekeepers for the minor round. One copy of this should be in the timekeepers box and one copy in the umpires room.
4. Send Off Form and Record
For completion by time keepers once a send off has occured. Form to be sent to the League with match day results envelope.

A brief report from the umpire to detail circumstance of a yellow card. Form to be returned to the League with match day results envelope
