All media individuals who wish to receive accreditation are asked to read and complete this application form. Email the completed form to media@2009pacificgames.co.ck

The deadline is Monday September 7, 2009.

Accreditation means holding an Identity Card issued after approval by Pacific Mini Games 2009 Limited (PMG). Accreditation allows the holder access to areas reserved for on-site media and entitlement to media services.

ACCREDITATION PROCESS
·       Each applicant fills in the accreditation form and returns it electronically
·       PMG will send an acknowledge receipt to the applicant. This receipt is NOT a confirmation of accreditation, merely that the application has been received.
·       In due time, PMG will confirm to the applicants the validity of the accreditation and give all details regarding media services and transportation.
·       Successful applicants must bring a form of identification to the Accreditation Centre (passport, driver’s licence, etc) to claim their accreditation pass.

NOTES ON COMPLETING THE FORM
Hardcopy, printed or postal applications will not be accepted.
All information must be given in English.

PHOTO
Please provide us with an electronic photograph by email to media@2009pacificgames.co.ck

The photograph must be saved in one of the following formats: JPG, TIF or PSD. The photograph should have a minimum resolution of 150 DPI and must be saved as:
Your last name + date of birth. JPG (example: smith16071960.jpg). No other format will be accepted. The front view (head and shoulders) photograph must be in colour with a white background. Hats, hair bands or dark glasses may not be worn, unless always worn for religious or medical reasons.  The photograph must not have been altered in any way.

CONDITIONS OF ACCREDITATION
No media accreditations will be issued after September 20, 2009. 
The process of submitting an application does not guarantee accreditation.
The bearer is responsible for keeping accreditations safe. Lost accreditations will be replaced for payment of a fee.
PMG may at any time and without notice refuse or withdraw accreditation on fair grounds, including violation of the security rules and guidelines. 
The press identification cards must be worn at all times while at all venues.
All media visiting the Cook Islands are responsible for their own visas to enter the country.
All media individuals who wish to receive accreditation must provide the information required in the accreditation form.
