If an injury occurs during a game please follow these procedures:

• Report your injury to the stadium office.  You should be given an injury report form to complete. If not, please request a form.
• The top copy of your injury report form stays with BBI.  You take the insurance claim application, complete it and send it with medical receipts to the insurance company.
