Life and Service Membership Nomination Forms
Kapi Mana Netball Centre would like to acknowledge and celebrate long-serving members of our netball community. We know there are a number of members who are deserving of recognition therefore we wish to encourage clubs and schools’ the opportunity to nominate members they think deserve recognition.
Please note that the Service Award and Life Membership award nominations are now open for 2014. If you know of someone who has made a valued contribution or service to netball, please take a look at the criteria and think about nominating them for these awards.
A Service Award nominee needs to have consistently given at least fifteen (15) years of continuous service to netball at Centre, Club or local level.
Life Member Award nominees need to hold the Kapi Mana Netball Centre Service Award prior to nomination for Life Membership for at least five (5) years of continuous service.
Nominations need to be received via email to centremanager@kapimananetball.org.nzby Friday 28 November 2014.
Please find attached nomination forms for each of these awards attached below.
Please be sure to gather and provide as much information as you can to enable the Service Award and Life Membership Committee to consider the nomination favourably and make a well-informed decision. The information will also be utilised during the awards presentation at the AGM.
