FNSW Incident Report

The Football NSW incident report has been updated in 2016.

An Incident report must be submitted for any misconduct occurring other than that causing you to show a player the Red Card, or sendind a coach from the technical area, in which case you should use a Send-Off Report.

All appointed match officials must write a sendoff report involving R1, R2, R3 and R6 or an incident report for all technical area dismissals and outside agent issues, even if you didn’t see or hear anything. Only the referee is required to write send-off reports for R4, R5 and R7.
