Country Association Affiliation- Deadline 1st October 2012
The 2012/13 WA Country Basketball season is nearly upon us! It is therefore time again for all Country Basketball Associations to affiliate.
Please find below the following affiliation documents:
- 2012/13 Affiliation Cover Letter
- Affiliation Benefits
- Form A- Association Affiliation (to be returned by October 1, along with $75 affiliation fee)
- Form B- Player Registration (to be returned by December 14)
- Form C- Registered Player Details (to be returned by 14th December- sent electronically only to zoe.hughes@basketballwa.asn.au)
All forms can be sent either electronically to zoe.hughes@basketballwa.asn.au or in the post to- Community Basketball Administrator, PO Box 185, Floreat, WA 6014.
Payments can be made by cheque (payable to Basketball WA) or by direct bank transfer (National Australia Bank, BSB: 086 488, A/C: 6354 87166)- please note when making a bank transfer please state your association name & the payment reason- i.e. Broome-Association Affiliation or Broome- Player Affiliation otherwise we will be unable to track your payment.
