Appointment of Guardian Form

Please use this form to appoint a temporary guardian to a minor at all AMCC practise days and events if a legal parent/guardian can not be present.
This form must be signed, dated, witnessed and handed in at sign in otherwise the minor will not be allowed to ride.

Recreational One Day Licence $30.00

All non-club members will need to join the club (this does not include our reciprocal clubs). PROOF OF RECIPROCAL CLUB MEMBERSHIP IS REQUIRED AND IS THE RESPONSIBILITY OF THE RIDER – NO PROOF = NO RIDE
