Please note that this form MUST be completed in triplicate.
One copy should be provided for the player's home club.
One copy should be kept for your records
The third copy MUST be forwarded to AFL Central Victoria Football Operations manager or placed in the match day documents.
This form MUST be completed prior to applying for the player through the online website system.
Once this form has been submitted to AFL Central Victoria Football there is NO necessity to complete the form each time your club applies for the particular player.
It is incumbent on your club to be fully conversant with the conditions relating to the use of any permit player.
