Volunteers and Coaches Police checks process for NEMFJA
- volunteer fills out the police check form - this form can be accessed online
- can either download a copy and fill in by hand or fill in online and print it off (preferred option)
- must attach copies of ID required to the form
- has to be signed by police/JP
- send all to Annette Cooper PO Box 21 St Agnes 5098 and I can process for free (if copies of ID not included and verified by police or JP then I will have to send it back to the Volunteer)
- must also be signed by the volunteer on page 2 at the top
- volunteer will receive police check back in the mail
- they then should take it to the club for verification of sighting
Police checks
A police check - 'National Police Certificate' provides a national summary of an individual's offender history. It is generally requested by organisations as one part of their process to ensure the integrity of their staff orvolunteers.
How to apply for a police check
Step 1:Download - complete online - print, a police check application

Alternatively you can obtain a printed copy from your localpolice station.
Only tick boxes on bottom of form for Voan Volunteer and Working with Children/Vunerable Groups
Step 2:Check that you have 100 points of original identification as explained in the application form.
Step 3:Get your application processed.
Attend apolice stationwith your:
- completed application form
- 100 points of original identification
- photocopies of 100 points of identification, for certification
- application fee.
South Australia Police (SAPOL) will then:
- witness your signature on your application form
- certify your 100 points of identification
- complete the authorisation section on your application form
- process your request within 10 - 15 working days.

See a Justice of the Peace with your:
- completed application form
- 100 points of original identification
- photocopies of 100 points of identification, for certification.
Request the Justice of the Peace to:
- witness your signature on your application form
- certify your 100 points of identification
- complete the authorisation section on your application form.
Post your completed application form to NEMJFA PO BOX 21 ST AGNES 5097
· NEMJFA Administrator will add the VOAN Number for free processing and send to:
